COVID-19 vaccine access update

USF Health is currently offering the COVID-19 vaccine to patients, employees and individuals in the community who are age 12 or older.

To receive a vaccine, you may either:
Schedule an appointment by logging into MyChart. If you do not have a MyChart account, please email covidvaccine@usf.edu to schedule your appointment. If you have login or other MyChart portal issues, please email mycharthelp@usf.edu for assistance.

Visit the Morsani Center COVID Vaccine clinic for a walk-in appointment. Please see below for clinic information.

Important Note

  • Appointments and walk-ins are available as supply allows.
  • For those age 12 to 17: The Pfizer vaccine is authorized for people age 12 or older.
  • The Moderna and Janssen (Johnson & Johnson) vaccines are authorized for people age 18 or older.
  • All individuals under age 18 receiving a vaccine must be accompanied by a guardian and complete the COVID-19 vaccine screening and consent form. To download a copy of the form, click here.

At USF Health, our priority is the health and safety of our patients and their families. We are closely monitoring the coronavirus (COVID-19) outbreak and are following the official recommendations provided by local and state departments of health.

During this time of uncertainty, we understand that the current COVID-19 crisis and the given local risk in Hillsborough County are concerning to many of our patients who are receiving care at our clinical locations. We are committed to improving health in the safest manner possible, and our health care providers are diligently working to take the necessary steps to safely provide critical clinical services to our patients and their families while protecting our staff.

COVID-19 Testing

USF Health currently offers the following COVID-19 tests. Your provider will determine which test is best for you based on your clinical symptoms and test kit availability.

While most insurance covers the cost of testing for COVID-19, those who don’t have insurance or who choose to self-pay can expect the following costs.

Antigen test: Infectious agent antigen detection by immunoassay technique, (e.g., enzyme immunoassay [EIA], enzyme-linked immunosorbent assay [ELISA], immunochemiluminometric assay [IMCA]) qualitative or semiquantitative, multiple-step method; 87426 severe acute respiratory syndrome coronavirus (e.g., SARS-CoV, SARS-CoV-2 [COVID-19]: $150 self-pay cost

Molecular test: Infectious agent detection by nucleic acid (DNA or RNA); 87635 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (Coronavirus disease [COVID-19]), amplified probe technique: $154 self-pay cost
