Whats wrong with this Picture?

Photographs are a learning tool of how accidents can be prevented. According to the Bureau of Labor Statistics’ Census of Fatal Occupational Injuries (CFOI) reported 54 fatalities occurred in the year 2009 from scaffolds, staging. In that study, 72% of workers injured in scaffold accidents attributed the accident either to the planking or support giving way, or to the employee slipping or being struck by a falling object. Finding: Scaffolding was erected on unstable foundation without mudsills.  

Hazards: Injury or death can occur as a result of an unstable scaffold.

Standard: 1926.451(c)(2) Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mudsills or other adequate firm foundation.

Corrective Action: Place scaffolding out of service. Have a competent person reinstall/erect scaffolding with mudsills.

Finding: Scaffolds were erected on 2×2 boards. Unstable objects shall not be used to support scaffolds or platform units.

Hazards: Injury or death as the result of an unstable scaffold

Standard: 1926.451(c)(2)(i): Footings shall be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement

Corrective Action: Place scaffolding out of service. Have a competent person reinstall/erect scaffolding with the correct footings that will support the load for the scaffolding.
